FILLING OUT THAT ENTRY FORM (for first time exhibitors)

Most entry forms are pretty basic. The show will ask for your name, address, phone number and email address . Some ask for an NPIP member number (if you have one).

The most often asked question has been, how do I list what birds I want to enter, how do I make out the entry form.

I want to enter 5 black cochins. I have 2 hens, a pullet, and the rest are cockerels. I have 10 Old English Game Bantams of different colors (varieties) I want to enter, 6 cocks, 2 cockerels, 2 pul­lets, and 5 pretty leghorns of different varieties, all cockerels.

We have made up a very basic sample entry form below and using the birds we want to enter
we have filled in the blanks. The forms you receive might look a little different from show to show but
they all are asking for the same information. Be sure to look for the entry deadlines & mailing
instructions.        ALWAYS make a copy of your entry form.
